Your identity needs to be confirmed.

You need to provide original documentation (no photocopies) with your application form.

Which documents do you need to provide?
You must provide at least ONE document from Group 1, plus any TWO from Group 1 or 2
If you can not provide ANY document form Group 1 you must provide 5 documents from Group 2

Passport any nationality	Marriage Certificate/Civil Partnership Certificate
UK Birth Certificate issued within 12 months of date of birth â€“ full or short form acceptable	Financial Statement e.g. pension, endowment, ISA, issued within past 12 months
EU Photo Identity Card EU countries only	Vehicle Registration Document (V5 or VC5)
Adoption Certificate (UK)	UK P45/P60 Statement issued within past 12 months
HM Forces ID Card (UK)	Mail Order Catalogue Statement documentation should be less than three months old
UK Firearms Licence	Bank/Building Society Statement documentation should be less than three months old
UK issued Driving Licence (Full or Provisional) England/Wales/Scotland/Northern Ireland; either photo card or paper. A photo card is only valid if the individual presents it with the counterpart licence	A document from UK Central/Local Government/Government Agency/Local Authority giving entitlement (UK) for example, from the Department for Work and Pensions, the Employment Service or the Customs & Revenue, documentation should be less than three months old
	Birth Certificate issued more than 12 months from date of birth
	Utility Bill electricity, gas, water, telephone â€“ inc. mobile phone contract/bill, documentation should be less than three months old
	Exam Certificate e.g. GCSE, NVQ, O'Levels
	TV Licence issued within past 12 months
	Addressed Payslip documentation should be less than three months old
	Credit Card Statement documentation should be less than three months old
	National Insurance Card (UK)
	Store Card Statement documentation should be less than three months old
	NHS Card (UK)
	Mortgage Statement issued within past 12 months
	Benefit Statement (e.g. Child Allowance or Pension) should be less than three months old
	Insurance Certificate issued within past 12 months
	Certificate of British Nationality (UK)
	Council Tax Statement (UK) issued within past 12 months
	Work Permit/Visa (UK) issued within past 12 months
	Court Claim Form documentation issued by Court Services, issued within past 12 months
	Connexions Card
	CRB Disclosure Certificate should be less than 12 months old
	Letter from Head teacher confirming identity should be less than 3 months old
	Asylum Registration Card
