PLEASE NOTE: TEAM OFFICIALS AND MATCH REFEREE's
All team and Match officials will have their CRB status checked by the
National Secretary PRIOR to being allowed to participate at ANY National event.
If the National Secretary does not hold details of your enhanced Disclosure
or details of when your application was processed by a BIPHA appointed
CRB countersignatory you will NOT be allowed to engage in ANY activities for
a team or act as a match official.

THE TWO DAY EVENT IS A COMMEMORATIVE FUND RAISING EVENT AS PART OF THE OWEN PRICE TRUST.
THE MORE TEAMS WHO PARTICIPATE THE MORE THE TRUST BENEFITS.
LARGER SQUADS MAY CONSIDER SPLITTING INTO TWO ?
FEES AND TIMES TO BE ARRANGED.

GAME/ PLAYER/PROGRAMME SPONSORS ARE BEING SOUGHT.
WHY NOT CONSIDER VOLUNTEERING AS PART OF THE TEAM OF GAME OFFICIALS.

30th June & 1st July - U12
7th & 8th July - U14
4th & 5th August - U10 & U21
In order for your region to qualify for an allocation
the NEC must have received the following by 31st January

All membership forms
Your regional membership database
All membership fees
All transfers and accompanying documentation
All official game sheets relating to BIPHA league games played up to and including 21st January

Application forms must be completed in BLACK ink.

Original Identification documents (as listed in the CRB applicants guide) must be included with your application

Incorrect applications will be returned (highlighting the errors) and a new form will be sent out for completion.

Applications submitted without the correct identification documents will be returned to the applicant

If you do not want to post your original identification documents arrangements can be made with your nearest CRB countersignatory
